Forms that create other forms?

I am a newbie, so I need some help with this... Here is what I want to do... <br>
I would like to create a master list of available fields that "party a" can select from Lets say they are:<br>
Name<br>
Address<br>
Email<br>
and <br>
Phone<br>
Party A wants Party B to fill out Name and Address so they click on Name and address. Then a form is genertated just with those 2 fields for Party B to fill out. <br>
What I would like to do is almost exactly like the form generator located at <br>
