Intermittent Inbound SIP

we have 2 SIP lines set up using 1 for inbound and one for outbound. the outbound works fine, the inbound works great.... some of the time.. some of the time, it wont send back an acknowledgement on the inbound, will not complete the call and the caller gets a busy signal. I havent done many SIP trunks, but this error is new to me.

the monitor shows this

Any thoughts ??
 
k. So that sip message is just an Options message, sent by your SIP Provider and used to check that the endpoint is still available.

If you can enable the SIP Call RX and TX on the sysmon and post a failed call here then I can run through it and see what you're replying. Could be a dodgy SIP ALG on your router/firewall or the configuration on the system. It's odd to have 2 different lines for Inbound or Outbound, rather than just 2 SIP Uri's on the same line.
 
Seems like a NATing issue. Do you have a firewall or SBC in between? are you using static or dynamic IP for your router's public IP? What is network topology tab on IPO set to?
