Outcalling to wrong number, correct in list

I've been working on an existing IP Office v4.2 and discovered a strange problem. One voice mailbox is an after hours emergency number where Outcalling has been turned on. When a message is left the VM Pro calls 5 numbers as has been set up for that extension with Phone Manager.

However, sometimes the system dials a wrong number that is off by only one digit from the correct one. It's as if there's a second profile for that extension that sometimes gets loaded. To make matters more interesting, the SMDR logs show that sometimes 10 digits are dialed, sometimes only 7.

I've looked at the profile in Phone Manager and all the phone numbers are correct. Are there any other places that contain Outcalling numbers specific to an extension?
